โค Survival & Health

Project Zomboid's health system is granular and realistic: each part of the body has its own health status. An untreated wound can become infected, a fracture immobilizes, and a hemorrhage is fatal if ignored.

โ˜
Bite = Almost Certain Death. A zombie bite infects with the Knox Virus in practically 100% of cases. The infection has no known cure and is fatal within 1โ€“3 game days. This is the PZ's main threat.

Body Parts and Injuries

Body PartPossible InjuriesImpact
HeadBruise, cut, lacerationsHeadache, possible unconsciousness
NeckScratch, Bite, LacerationsMaximum risk of Knox infection; severe bleeding
TorsoContusion, Cut, Puncture (bullet)Possible internal bleeding, pain that affects performance
ArmScratch, Bite, Fracture, CutFracture โ†’ immobilization of the arm; affects combat and items
HandCut, ScratchPenalty for manual actions (crafting, combat)
LegBite, Scratch, Fracture, SprainFracture โ†’ severely limp; twist โ†’ limping
FootCutting, TwistingReduced mobility

Types of Injury and Chance of Knox Infection

WoundCauseKnox InfectionImmediate Treatment
Scratch Contact with zombies 7% Disinfectant + Bandage
Laceration More intense contact with zombies 25% Disinfectant + Bandage (urgent)
Bite Zombie bite ~100% There is no treatment for the virus
Cut Broken glass, fall, tool 0% Disinfectant + Bandage or Suture
Deep Wound Knife, shrapnel, large glass 0% Suture (needle + thread) + Preventive antibiotics
Fracture Fall from height, collision 0% Splint (splint) + rest (days)
Sprain Slight fall, sudden movement 0% Splint + rest (hours)
Burn Fire, explosion 0% Special bandage, do not expose to the sun
Bullet Wound Shooting (PvP or accidental) 0% Tweezers โ†’ remove bullet โ†’ Suture โ†’ Antibiotics

First Aid Protocol

  1. Stop and Assess: Open the health menu (H key by default) and identify all active injuries.
  2. Disinfect: Apply Alcoholic Wipes, Rubbing Alcohol or any alcohol to the wound. This prevents bacterial infection.
  3. Bandage: Cover with Bandage, Ripped Sheets or Cotton Balls. Uncovered wounds bleed and become infected more quickly.
  4. Suture (if necessary): Deep wounds require a needle + suture thread (Needle + Thread) or Suture Needles to close.
  5. Monitor: Change dirty bandages (red icon in health interface). Dirty dressings increase the risk of bacterial infection.
  6. Rest: Wounds heal faster by sleeping. The healing rate is significantly higher during sleep.

Knox Virus โ€” The Main Threat

The Knox Virus is the cause of the apocalypse in Project Zomboid. When your character becomes infected, death is practically inevitable.

Progression of Knox Infection

Phase 1 (0โ€“6h)
Incubation: No visible symptoms. You may not know you are infected. The character is completely normal.
Phase 2 (6โ€“24h)
First Symptoms: Mild fever, increased tiredness, occasional nausea. It can still be confused with the flu or food poisoning.
Phase 3 (24โ€“48h)
Advanced Symptoms: High fever, tremors, generalized pain. The character becomes significantly slower and weaker. Severe stamina drop.
Phase 4 (48โ€“72h)
Terminal Phase: The character periodically loses consciousness. Imminent death. No cure possible.
โš
Fever โ‰  Always Knox: Fever can be caused by a bacterial infection (treatable with antibiotics) or by the Knox Virus (untreatable). Watch for progression โ€” bacterial infection improves with antibiotics. If the fever gets worse even with antibiotics, it's likely Knox.

What to do if you get a bite

  1. Accept the situation โ€” most bites are fatal in Apocalypse mode.
  2. Organize your camp: Leave your base organized for the next character, if playing with inheritance.
  3. Distribute important items in known locations to facilitate future looting.
  4. Take advantage of the last few days to clean areas close to your base (the work is not wasted).
  5. Consider adjusting the Sandbox settings to reduce ZombieTransmission if the bites seem unfair.
๐Ÿ’ก
Sandbox Configuration: In ZombieTransmission, you can reduce the chance of transmission through bites and scratches. Useful for those who find the mechanics too punishing at first.

Painkillers Reduces pain. Removes performance penalty for pain. Fractures, serious injuries causing slowness. It doesn't cure โ€” it just masks the pain. Do not overdo the dosage.
Beta Blockers Reduces stress and panic drastically. Before intense combat, in high-stress situations. Addiction with frequent use. Use sparingly.
Sleeping Tablets Induces deep sleep quickly. When tired but in a safe place and needing to sleep. It can make you sleep longer than planned. Never use in an unsafe location.
Vitamins Improves general health, boosts immunity. Prevention. Take regularly with good nutrition. Light effect โ€” does not replace active treatment.
Anti-depressants Reduces unhappiness permanently while active. Characters with high unhappiness penalties. Withdrawal effect when stopping โ€” temporary worsening of mood.
Water Purification Tablets Purifies dirty/contaminated water. Whenever you need to drink water from unreliable sources. Recipe: 1 tablet + container with water = purified water.

Diseases & Conditions

Disease Reference

Disease / ConditionCauseSymptomsTreatment
Bacterial Infection Untreated wound, dressing dirty for a long time Fever, wound pain, redness Antibiotics + Dressing change + Disinfection
Knox Virus Zombie Bite (100%), Laceration (25%), Scratch (7%) Progressive fever, weakness, tremors โ€” no improvement No cure. Only symptomatic with Painkillers/Vitamins.
Flu / Cold Exposure to cold and rain without suitable clothing Sneezing, coughing, mild fever, fatigue Rest + Vitamins + Staying warm. Passes in 1โ€“3 days.
Food Poisoning Spoiled food, raw when it should be cooked, poisonous mushrooms Nausea, vomiting, diarrhea, severe weakness Rest + Hydration + time. Severe cases: hospitalize.
Dehydration Don't drink water for a long time Extreme thirst, weakness, foggy thinking Drink water immediately. Severe cases are lethal.
Hypothermia Very low body temperature (cold + wet) Tremors, extreme slowness, drowsiness Immediate heating: fire, dry clothes, sleeping covered.
Hyperthermia Very high temperature (heat + heavy clothing) Excessive sweating, dizziness, weakness Remove clothing, shade, cold water, AC or ventilation.
COโ‚‚ Poisoning Generator in closed space Progressive drowsiness โ†’ unconsciousness โ†’ death Prevention: always generate the generator outdoors. No cure after unconsciousness.
Sprain / Torsion Fall, sudden movement, running on uneven surfaces Limping, pain when walking Splint + rest. Improvement in hours to 1 day.
Bone Fracture Fall from height (BoneFracture=true), car collision Severely limping, unable to use limb Mandatory split + rest for several days. Comfrey Cataplasm accelerates.
๐Ÿ’ก
Distinguishing Knox Virus from Flu: Both cause fever. The difference: flu improves with rest and vitamins in 1โ€“2 days. Knox virus progressively worsens and does not respond to antibiotics. If you have recently been bitten or lacerated and have a fever, assume Knox.

Nutrition & Food

With Nutrition activated (standard in Apocalypse), your character needs to balance four macronutrients. Imbalance causes strength and endurance penalties.

The Four Macronutrients

NutrientFunctionDeficiencyExcess
Calories General energy, controls hunger bar Weakness, weight loss, low stamina Weight gain if much above needs
Proteins Muscular strength, injury recovery, muscle building Loss of strength, slower healing, reduced stamina Excess rarely problematic
Carbohydrates Quick energy, fuel for intense physical activity Rapid fatigue during physical exertion Weight gain, possible increase in triglycerides
Lipids Long-lasting energy, overall health General weakness, reduced immunity Excessive weight gain with too much excess

Nutritional Sources by Macro

๐Ÿฅฉ High in Proteins

Meat (any type), fish, eggs, canned beans. Essential for maintaining strength and healing injuries quickly.

๐ŸŒพ High in Carbohydrates

Bread, pasta, rice, potatoes, corn, oats. Fuel for intense physical activity and running.

๐Ÿงˆ High in Lipids

Cheese, butter, nuts, olive oil, bacon. Long-lasting energy โ€” important for days of constant activity.

โšก High Calorie

Chocolate, peanut butter, processed food, cheese. To regain weight after illness or lean periods.

Nutrition States and Their Effects

StateDescriptionEffect on Character
StarvingExtreme hunger โ€” empty hunger barSevere weakness, zero stamina, impossible to run
HungryHunger โ€” bar below 50%Slight performance reduction
FullSatiated โ€” bar at ideal levelNormal performance
OverfedOverfed โ€” bar too highSlight sluggishness, stomach discomfort
UnderweightBelow healthy weightReduced strength and stamina, more fragile
OverweightOver healthy weightSlower running, reduced stamina
ObeseVery overweightSeverely reduced stamina, compromised mobility
๐Ÿ’ก
Eating Strategy: For the first few days, eat whatever you can find. From week 2, start planning โ€” balance meals with protein (meat), carbohydrates (bread/rice) and lipids (cheese/butter). The Nutritionist trait shows the exact values of each food.

Stress & Panic

Stress (unhappiness) and Panic are two separate systems that affect the character's performance in different ways.

Stress (Unhappiness)

What increases StressWhat reduces Stress
  • Killing zombies (stacks over time)
  • Being injured for a long time
  • Being hungry or thirsty
  • Being very dirty/bloody
  • Witness the death of allies (multiplayer)
  • Sleep deprivation
  • Eating spoiled or bad food
  • Running out of cigarettes (Smoker trait)
  • Read books (any book/magazine)
  • Watch TV (any program)
  • Listening to the radio (especially music)
  • Taking a shower (cleans and reduces stress)
  • Smoking cigarettes (short term)
  • Drinking alcohol (short term, hangover later)
  • Sleep well in a safe place
  • Beta Blockers (medicine)
  • Eat delicious/cooked food properly
  • Anti-depressants (long term)

Panic

Panic is different from Stress โ€” it is an acute response to threatening situations. Causes gun sight tremors and combat penalties.

What causes panicHow to reduce it
  • Being surrounded by lots of zombies
  • Zombies very close to the character
  • Hearing zombies without seeing them (surprise)
  • Running in panic for a long time
  • Cowardly trait (easier to panic)
  • Eliminate nearby threats
  • Flee to a safe place
  • Beta Blockers (immediate reduction)
  • Brave trait (less susceptible)
  • Desensitized trait (almost immune)
  • High Aiming skill (less shaking)

Sleep & Tiredness

Tiredness (fatigue) increases with physical activity and time spent awake. Very tired characters become slower, less accurate and make more noise.

Effects of Fatigue by Level

Tiredness LevelEffects
RestedMaximum performance. No penalty.
Slightly TiredSlight reduction in stamina. Imperceptible in practice.
TiredReduced stamina, slower movement, slower reactions.
Very TiredVery low stamina, slow movements, unstable aiming.
ExhaustedAlmost impossible to fight or run. Extreme danger.
Sleeping Standing UpCharacter may faint. Critical situation โ€” go to sleep immediately.

Sleep Management Tips

  • Safe Place Always sleep in a closed and safe place. Barricade windows and doors before bed.
  • Schedule Sleeping during the day in a safe place is just as effective as at night. Consider sleeping during the day in places you've explored and scavenged.
  • Sleeping Tablets They induce rapid sleep, but can make you sleep longer than planned. Use only in very safe locations.
  • Night Owl Trait Needs less sleep โ€” excellent for maximizing productive time.
  • Sleepyhead Trait Needs more sleep โ€” requires comfortable beds and safe places more often.

Temperature & Climate

The temperature system directly affects the character's health and performance.

Factors that Affect Body Temperature

FactorEffect on Temperature
Ambient temperature (seasons)Summer = hot; Winter = cold
RainQuickly cools โ€” wet + cold = hypothermia
ClothesHeavy clothes heat up in winter, overheat in summer
Physical activityRunning and fighting warms up the body
Fireplace / FireplaceHeats significantly
IndoorsProtects from wind and rain, more stable temperature
Heated vehicleHeats up quickly

Clothes Guide by Season

โ„ Winter / Cold

Wear multiple layers: t-shirt + sweater + coat. Boots and gloves are important. Stay dry โ€” wet clothes lose warmth.

โ˜€ Summer / Heat

Light clothes (t-shirt, light pants). Avoid heavy coats โ€” they cause hyperthermia during physical activity. Seek shade and water.

๐ŸŒง Rain

Waterproof clothing (trench coat, raincoat) reduces cooling when wet. Important in winter to prevent hypothermia.

๐Ÿ‚ Autumn / Spring

Variable temperature. Carry an extra layer in your backpack. Be flexible as the temperature changes throughout the day.

Hygiene

Dirt and blood on a character affects unhappiness and can influence how zombies perceive him by smell.

Sources of Dirt

  • Blood Accumulates from killing zombies or being injured. High blood levels increase stress and can attract zombies by smell.
  • Dirt Accumulates over time, especially when working outside.
  • Dirty Clothes Clothes accumulate dirt regardless of the character.

How to Clean

  • Bath Use a bathroom with a shower (while the water lasts) or a bathtub. Removes blood and dirt from the character.
  • Improvised Washing Bucket of water + Soap allows you to clean yourself anywhere.
  • Washing Clothes Washing machine (with electricity) or hand wash with water + soap.
  • Rain Standing in the rain slowly reduces dirt (but cools the character).

Combat & Zombies

Project Zomboid's zombies are individually simulated with their own alertness, vision and hearing. Understanding how they work is essential to survive.

How Zombies Detect You

SenseBase RangeAmplifying FactorsHow to Reduce
๐Ÿ‘ Vision Medium-Long Sunlight, flashlights, fire light, vehicle lights Stay crouched, move slowly, no lights, Cat's Eyes (trait)
๐Ÿ‘‚ Hearing Very High Gunshots, engines, radios, alarms, running, screams Walking (not running), silent shoes, Graceful (trait)
๐Ÿ‘ƒ Smell Low Excessive blood on the character, corpses nearby Keep clean, use soap

Combat Strategies

๐Ÿคซ Escape & Stealth

Squat (C) to reduce noise and visibility. Lure zombies with sounds away from areas you want to clean โ€” throw a bottle or turn on a radio in a different room.

โš” 1v1 Combat

Use the push attack (Right Button) to create distance before striking. Never fight large groups in melee. One zombie at a time is the golden rule.

๐Ÿ”ซ Firearms

Extremely effective but very noisy. A single shot can attract dozens of zombies from the entire neighborhood. Use only with a planned and known escape route.

๐Ÿ”ฅ Fire

Extremely effective for hordes but extremely dangerous โ€” fire spreads to buildings (FireSpread=true). Use only in open spaces away from your base.

๐Ÿš— Vehicle

Running over zombies is effective but damages the vehicle. Use only in emergency situations or against isolated groups on roads. Speed + right direction = powerful weapon.

๐Ÿ  Construction Cleaning

Clean buildings room by room, close doors after each room. Never open a door without checking that there is room to retreat. Traps at the entrance help control flow.

Hand-to-Hand Combat Mechanics

  • Push Right click while weapon equipped. Push the zombie to create distance. Essential to avoid bites.
  • Kill Move The final kill animation. Occurs automatically when zombie is downed and you get close. It consists of stepping on the head or stabbing.
  • Swipe Some weapons have a swing area that hits multiple zombies. Bats, shovels and axes have a wide arc.
  • Knockdown Chance to knock down the zombie when hitting. Knockdown is very useful โ€” zombies on the ground can't bite and you can apply kill moves.
  • Beware of Stamina Fighting consumes stamina. With low stamina, attacks are very slow and you are vulnerable. Never enter combat with low stamina.

Stealth

Well-executed stealth allows you to move through densely infested areas without having to kill every zombie. The Sneaking ability reduces the noise of footsteps; the ability Nimble allows you to move faster in combat stance.

Factors Affecting Stealth

FactorImpactControl
Crouch (C)Reduces visibility and noise significantlyAlways use when infiltrating
Shoe TypeLeather = noisy; Sneakers = quietWear soft shoes for exploration
Floor SurfaceCarpet = silent; Wood = medium; Concrete = loudPlan route avoiding noisy surfaces
SpeedRunning = very noisy; Floor = medium; Crouching = silentReduce speed in critical areas
Skill SneakingReduces the noise of footsteps in all contextsTrain by walking crouched near zombies
Graceful TraitReduces noise from footsteps in generalTake at the beginning of the character
Inconspicuous TraitReduces chance of being spottedTake at character start
๐Ÿ’ก
Decoy Technique: Carry a portable radio on. When you want to distract zombies from an area, leave the radio on in a strategic location and they will investigate the noise, clearing your path.
